After the waiting period, if you are treated or diagnosed with a covered critical illness event you are eligible to receive a lump sum single cash benefit under your critical illness insurance plan. The benefit is payable upon initial diagnosis of a covered critical illness, such as:

Heart Attack Stroke
Major Human Organ Transplant End-Stage Renal Failure

Recovering from one of these illnesses take a toll on yourself and your loved ones. Having this type of coverage will allow you to focus on getting better and not worrying about how you will pay for your co-pays, deductibles and other out-of-pocket expenses. This benefit is in addition to coverage through your medical or disability plans and can be used for any expenses you deem appropriate.

Coverage Amounts for New Hires or During Special Enrollment Period (March 1, 2012 – May 31, 2012):
Employees: $5,000 – $50,000; $20,000 Guaranteed Issue
Spouses/Domestic Partners: $2,500 – $25,000; $10,000 Guaranteed Issue
Dependent Children Coverage: 25% of employee amount at no additional cost

In addition to coverage for a covered critical illness, this program also provides an annual Health Screening benefit to employees, spouses  and domestic partners.
